Acute nausea in Berlin — a symptom with many causes
Nausea is a non-specific symptom with a broad differential. The most common cause in Berlin is viral gastroenteritis, followed by migraine, vestibular disorders, drug side effects and pregnancy nausea. But nausea can also be the first sign of myocardial infarction — particularly in women and older patients — or of incipient sepsis, diabetic ketoacidosis, acute glaucoma, pancreatitis or raised intracranial pressure.
On the house call we take time for the structured history and exam that work through these differentials. We measure vital signs, record an ECG with cardiac signs, examine the abdomen, screen neurologically and decide with you on the right path.
What happens during the house call
We ask about onset, character, accompanying symptoms (abdominal pain, headache, chest pain, dizziness, fever, vomiting), medication, comorbidities and travel. We measure blood pressure, pulse, temperature, respiratory rate and oxygen saturation. For any unclear nausea from middle age onward we record an ECG to avoid missing atypical cardiac ischaemia. We examine the abdomen structurally (tenderness, rebound, percussion, bowel sounds), rule out cholecystitis, pancreatitis or appendicitis, screen neurologically. With classical nausea without red flags we administer an antiemetic (metoclopramide, ondansetron or dimenhydrinate) by injection or IV and plan further work-up by finding.
When imaging or hospital is appropriate
For unclear nausea with abdominal pain, with markedly elevated inflammatory parameters, with suspected surgical disease or with persistently reduced general state we recommend prompt further work-up — ultrasound is not feasible on the house call. We issue the referral, organise hospital admission if needed and prepare a written report.
How the house call works
On first contact we screen for red flags of cardiac, neurological or surgical cause. With clear emergency signs we direct you to 112 or hospital. Otherwise we dispatch the next available specialist — typically a 60- to 90-minute arrival.
On site we perform the structured exam, record an ECG if indicated, administer an antiemetic and treat symptomatically. If needed we place an IV. You receive written escalation criteria and a referral as appropriate.
Billing and insurance
We bill according to the German private medical fee schedule (GOÄ) via our Privatärztliche Verrechnungsstelle. House call, internal medicine exam, ECG, IV fluids and administered medication are itemised separately. German private health insurance and Beihilfe schemes typically reimburse these positions in full.
We can issue an English invoice on request and remain reachable by phone for billing questions after the visit.

Can the doctor give something for nausea right away?
Yes. An injection of metoclopramide, ondansetron or dimenhydrinate typically takes effect within 10 to 20 minutes.
Must I call a doctor for every episode of vomiting?
No. With a clearly viral gastroenteritis with mild disease and good general state, observation with oral fluids usually suffices. With prolonged persistence, poor tolerance or red flags it is worth calling.
Do I always need an ECG?
Not always, but with unclear nausea from middle age onward or cardiac risk factors yes — atypical infarction symptoms are easily missed otherwise.
Can I take antiemetics in pregnancy?
Yes, there are pregnancy-safe agents such as doxylamine/vitamin B6 or meclozine. We choose carefully by trimester and individual constellation.
