Fever in Berlin — a structured workup at home

In adults we speak of fever from 38.5 °C and high fever from 39.5 °C. Children have slightly different thresholds — and in infants under three months any temperature from 38 °C is reason for prompt medical assessment. In older patients an apparently moderate fever combined with confusion or weakness can also be an important warning sign.

The house call has clear value here. Our specialists bring the front-line diagnostics that matter — and they take time to narrow down the cause in a structured way, instead of issuing rapid labels under clinic pressure. A febrile patient often belongs in bed, not in a waiting room — with medical care delivered on the spot.

Structured workup of fever

We examine systematically for the most common sources: lung auscultation (pneumonia?), throat and tonsil inspection (tonsillitis?), otoscopy (otitis media?), abdominal palpation (cholecystitis, appendicitis, pyelonephritis?), lymph nodes, skin (rashes, erysipelas, shingles?), meningism. Pulse oximetry and a CRP rapid test help grade severity; rapid tests for influenza, COVID-19, streptococcus and urine help clarify the pathogen. On this basis we discuss the next therapeutic step with you.

Fever in children — calm and clear criteria

Children fever more often and more highly than adults; this is physiological and usually not a cause for concern. What matters is the clinical impression: how is the child drinking, how does the child respond, are breathing, colour and consciousness unremarkable? Our physician examines in a child-friendly way, hands you clear escalation criteria and decides with you on rapid tests or onward paediatric review where indicated. Febrile seizures are often frightening but in most cases brief and self-limiting — prolonged or focal seizures require hospital care.

How the house call works

On first contact we clarify the duration, height and trajectory of the fever, accompanying symptoms, comorbidities and medication. If anything suggests an immediately threatening course we direct you straight to the nearest suitable hospital. Otherwise we dispatch the next available specialist.

On site we perform the structured examination, add rapid tests and discuss therapy. If oral antipyretics are insufficient or absorption is unreliable, we can administer fever-reducing medication intramuscularly or intravenously. We issue prescriptions and a sick-leave certificate as appropriate.

Billing and insurance

We bill according to the GOÄ via our Privatärztliche Verrechnungsstelle. House call, examination, rapid tests and administered medication are itemised separately. German private health insurance and civil-servant Beihilfe schemes typically reimburse these positions in full.

We issue an English invoice for international guests on request and remain reachable by phone for billing questions after the visit.

Frequently asked questions

From what temperature should I call?

Adults: fever above 39 °C unresponsive to standard medication, or any fever with red flags such as breathlessness, severe headache or confusion. Infants under three months: always from 38 °C.

Can the doctor reduce the fever directly?

Yes. Beyond oral medication we can administer fever-reducing drugs intramuscularly or intravenously when needed — onset is faster than with oral therapy.

Should I go to the ER with a fever?

Usually not. ER is reserved for life-threatening situations. A house call efficiently clarifies whether a serious cause is present and what level of treatment is appropriate.

How long can a fever last?

Viral infections typically run for three to five days. If fever persists or new symptoms appear, repeat medical assessment is sensible.
