Acute neck pain

Acute neck pain in Berlin — when the house call provides relief

Most acute neck pain is muscular-functional — the result of posture at the desk, awkward sleep, drafts or a sudden jerky movement. In daily Berlin practice we see many patients who can barely turn their head in the morning after intense home-office days or an uncomfortable flight. The pain often radiates into the back of the head, shoulders or hands.

In most cases a careful clinical examination is the decisive step. We check for neurological deficits, whether the symptoms fit a pure cervical syndrome or whether rare but important differentials — disc herniation with nerve-root compression, meningitis, vertebrobasilar circulatory disorders — need to be considered.

What happens during the house call

We ask about onset, triggers, character, radiation and accompanying symptoms (headache, dizziness, visual loss, numbness in arms or hands). We examine mobility, pressure pain, upper-limb reflexes, sensation in dermatomes C5 to T1 and muscle strength. With a classical cervical syndrome without neurological deficits we use gentle manual techniques, administer an intramuscular analgesic (diclofenac, metamizole) and advise on heat or positioning. With radicular symptoms with deficits we propose MRI and specialist input. We issue prescriptions, sick-leave certificate and a physiotherapy referral on request.

What you can do yourself

Move the head carefully within the pain-free range — strict rest worsens guarding. Warmth (cherry-pit cushion, warm shower) usually helps more than cold. A rolled towel as a neck pillow when lying on your back eases the way to sleep. Raise screens to eye level, take short movement breaks. For recurring courses, posture and workstation analysis plus physiotherapy is the most sustainable approach.

How the house call works

On first contact we clarify onset, triggers, accompanying symptoms and comorbidities. With signs of meningitis, subarachnoid haemorrhage or relevant cervical trauma we direct you to 112 or hospital. Otherwise we dispatch the next available specialist — typically a 60- to 90-minute arrival.

On site we perform the structured neurological and musculoskeletal exam, treat manually and with analgesics where indicated, and demonstrate exercises and positioning. You receive prescriptions, sick-leave certificate and, if needed, a physiotherapy referral.

Billing and insurance

We bill according to the German private medical fee schedule (GOÄ) via our Privatärztliche Verrechnungsstelle. House call, examination, manual-therapy elements and administered medication are itemised separately. German private health insurance and Beihilfe schemes typically reimburse these positions in full.

We can issue an English invoice on request and remain reachable by phone for billing questions after the visit.

Frequently asked questions

Do I need an MRI?

Not immediately. For classical cervical syndrome without neurological deficits, clinical exam suffices. We recommend MRI with radicular symptoms with deficits or a persistent course.

Can the doctor use manual therapy?

Yes. With clear indication and no red flags we use gentle manual mobilisation. With any more serious picture we prefer conservative therapy and specialist input.

Heat or cold?

For muscular neck pain, warmth usually helps more. Cold can be useful with an acute inflammatory component. Use what works for you.

Can the doctor sign me off work?

Yes. We can issue a sick-leave certificate on site and add a physiotherapy referral if needed.
